TERMS AND CONDITION FOR GIFTS

Fill up the gift request form following the instructions attached to the form. You can do this only after your dish connection has been installed as well as activated. When we receive your filled up form, we will send you a gift voucher via mail. The address of the mail will be that of the account holder during installation. You have to return the form to the address listed on the envelope. Your account needs 60 to 120 days to activate. The free gift voucher can only be yours when you have the required documents of the payments made to The American Dish TV.

Your gift will be with you within a time period of 6-8 weeks. It would not be a good idea to send out the S&H fees before you have received the free gift coupon. The offer is valid for U.S. residents who have a correct mailing address.

The shipping as well as handling fees for the MP3 Players, DVD Player or the Home Theater will be $14.95. This offer is not valid on reprinted forms or in areas where it is prohibited by laws. The minimum packages that you need to have to avail this offer are America's Top 100, Dish Latino or any other package that is over the mentioned two.

Only one household per customer can avail this offer. If your desired gift is not available at that point of time, you will receive some alternative gift. You have to be registered after August 1, 2006 and June 20, 2009. Within 21 days of hardware arrival, your satellite TV will be activated. The customer has to keep their dish TV services active for 180 days. If not, you need to pay $150 to the company.
